I.The Premise

Sleep is not the time between days. It is the time that builds them. The hours your body uses to repair its tissue, consolidate what you learned, clear the protein debris from your brain, regulate the hormones that decide your weight, your fertility, your mood, your immune response, and the rate at which you age.

Skip a night and you feel it. Skip a year of decent nights and the bill comes due — in heart disease, in metabolic dysfunction, in cognitive decline, in years off your life. The most consequential third of your existence runs almost entirely unmeasured.

We think that's strange. So we built a program to fix it.

Most sleep is invisible. Most sleep problems go undiagnosed.

Insomnia care in America has historically failed in two directions — Z-drugs handed out indefinitely, or medication treated as something to be ashamed of. Our position is older and quieter. Sleep medication can be a bridge, occasionally a destination, and the right choice depends on which kind of insomnia you have. We do not start patients on benzodiazepines or Z-drugs as first-line therapy. We use safer modern hypnotics, the cheap old workhorses that still work, and short courses when appropriate.

Dual orexin receptor antagonists — the modern first-line for chronic insomnia. They quiet the brain's wake-promoting orexin system rather than sedating broadly, preserve normal sleep architecture, and carry low dependence risk. FDA-approved for both sleep onset and maintenance.
Low-dose Trazodone
25–100 mg · Off-label
An older serotonin modulator used at low doses for sleep maintenance. Inexpensive, generic, non-habit-forming, well-tolerated long-term. The reliable workhorse — particularly useful when insomnia coexists with low mood or anxiety.
Low-dose Doxepin
3–6 mg · Sleep maintenance
An old tricyclic given at micro-doses — too low for antidepressant effect, just enough to block histamine for sleep maintenance. No next-day grogginess, no dependence, generic-cheap. Useful for the patient who falls asleep easily but wakes at three.
Ramelteon
Melatonin receptor agonist
A targeted melatonin-receptor agonist for circadian and sleep-onset insomnia, particularly useful in older patients and shift workers. Non-scheduled, non-habit-forming, no rebound insomnia on discontinuation.

Treat without drugs.

Memory consolidation, emotional regulation, dreaming. Loss of REM sleep is one of the strongest correlates of cognitive decline.

Transitional and connective. The bulk of the night and the gateway between deep restoration and REM processing.

Brief arousals are normal. Frequent ones — especially if patterned — point at apnea, anxiety, or environmental disruption.
