Insomnia care in America has historically failed in two directions — Z-drugs handed out indefinitely, or medication treated as something to be ashamed of. Our position is older and quieter. Sleep medication can be a bridge, occasionally a destination, and the right choice depends on which kind of insomnia you have. We do not start patients on benzodiazepines or Z-drugs as first-line therapy. We use safer modern hypnotics, the cheap old workhorses that still work, and short courses when appropriate.
